How to Complete the First Home Owner Grant Application Form
The First Home Owner Grant (WA) (FHOG (WA)) application form is to be completed by all persons with a financial interest in the property before being lodged with either the FHOG approved agent or the Office of State Revenue
FHOG Approved Agents
If the financial institution that you are purchasing your first home through is a First Home Owner Grant (WA) approved agent they can accept and process your application on your behalf and in doing so you may receive payment faster. Office of State Revenue
You will need to deal directly with the Office of State Revenue if your lender is:
- Not an approved agent.
- You wish to apply independently of your lender or
- You are not borrowing for the purchase of your home
Required supporting documentation
If making application through the Office of State Revenue the following documentation is mandatory:
- Proof of identity.
Every applicant must provide an original or certified copy of the document in four separate categories. The same single document must not be used in more than one category.
The preferred documents are shown below. If you can’t provide the preferred documents, you can contact the Office of State Revenue to discuss your particular situation. Only post certified copies by mail do not send originals.
Category 1 – Primary identity:
Australian Citizen:
- An Australian birth certificate.
- Australian Passport.
- Australian Certificate of Citizenship.
New Zealand citizen:
- Current passport
- NOTE: New Zealand citizens must be living in Australia when making application.
If a citizen of another country:
- Current passport.
- Evidence of visa conditions.
Category 2 – photo and signature:
- A current Australian Drivers Licence or…
- A current Passport or…
- A current Firearms Licence or …
- A photo ID card issued by Consumer Affairs or…
- A certified photocopy of a photograph accompanied by a certified declaration explaining why none of the previous ID’s are available.
Category 3 – Evidence of Australian residency:
- A Medicare card.
- Motor vehicle registration.
- Veteran Affairs card.
- Centrelink card.
- Debit or credit card.
- Other evidence such as can be found here Proof of Identity form.
Category 4 – Proof of residential address:
- Electricity, gas, water bills.
- Insurance policy.
- Bank account or equivalent.
- Other evidence such as can be found here Proof of Identity form.
Supporting evidence will also have to be submitted with the application if married (copy of marriage certificate); divorced (copy of divorce certificate); widowed (copy of death certificate); name change (copy of certificate showing change of name); separated (statutory declaration).
- Proof of Transaction:
- Copy of contract to purchase an established home, a new home or one off-the-plan.
- Copy of contract to build.
- Documents giving evidence of the building having been either commenced or completed.
